Treatment
Doppler Guided Hemorrhoid Artery Ligation (DGHAL), one of the best piles treatment for all types of piles (hemorrhoids), is a painless procedure which is sometimes preferred over many other treatment options available at our piles clinic in Mumbai.
This procedure is specially indicated in piles patients who present with bowel incontinence, those who are at risk of having post hemorrhoidectomy incontinence and individuals who prefer painless treatment over painful surgeries which may require long hospital stays.
On the other hand, the main contraindications for DGHAL have been identified as the usage of anticoagulants such as Aspirin and Coumadin which may increase the risk of bleeding following the procedure. However, this is just a relative contraindication which can be intervened according to the comorbidities of the patients.
Technique of DGHAL
1-2 Fleet enemas will be provided few minutes before the process to empty bowel.
Intravenous anesthesia known as Propofol is administered in order to sedate the patient prior to the procedure followed by local anesthesia around the site of rectum.
A special instrument known as Proctoscope attached to a rotatable needle is pushed into the rectum by which causative hemorrhoid arteries are identified, isolated and ligated using stitches.
As a result, blood supply to the anal cushions (piles) will be cut off leading to their sloughing off or shrinkage later.
Furthermore, the stitches used to ligate these blood vessels will form a rectal pexis (web-like) by connecting the rectal mucosa to the layers underneath, thereby raising the hemorrhoidal cushions up above the dentate line which will reduce the risk of rectal prolapse.

However, patients may complain of an urge to empty bowel without any resultant defection , following the operation and this can last for 12-24 hours. As far as the other possible complications are considered, some patients may present with mild bleeding immediately after the procedure which may be mostly due to the use of Aspirin.
A minority of patients may develop infections which can lead to generalized sepsis, perianal thrombosis and anal fissures. But, sufficient precautions and early identification of symptoms associated with likely complications can prevent the occurrence of serious conditions later on. There can also be a very low chance of recurrence which can occur in only about 5% of the patients.
