Manometry is measuring the pressure inside different parts of gastrointestinal tract, by entering a catheter with transducers. These transducers will travel through the mouth or anus into the lumen of the particular organ which needs to be investigated.
This procedure is usually carried out to diagnose various GI motility disorders and can be used in sites including esophagus, stomach, duodenum, sphincter of Oddi, and rectum.
What is esophageal manometry?
This is a procedure, mainly indicated in patients who have symptoms like dysphagia, chest pain, heartburn or regurgitation.
The individual pressures in the upper and lower esophageal sphincters will be measured separately and this will help to reveal the efficient coordination of peristaltic movements, to diagnose any abnormal contractions.
Various esophageal motility disorders including Achalasia cardia, Systemic sclerosis and diffuse spasms can be diagnosed this way. It can also be used to assess the functioning of esophagus and structural abnormalities like ‘hiatus hernia’ prior to certain therapeutic methods like anti-reflux surgery.
Additionally, a newly introduced method of high-resolution manometry is used nowadays in combination with impedance testing in order to find out the efficiency of bolus transition through the esophagus simultaneously during the swallowing test.
Procedure:
Patient is seated and a topical anesthetic cream is applied in the inner aspect of the nose to numb the area.
Thereafter a tiny, flexible tube (catheter) is entered through the nose which will pass down the esophagus into the stomach.
Then, the patient is asked to lie on to his left side and the proximal end of the tube is connected to an instrument which measures the pressure in the lower esophageal sphincter and esophageal muscles.
The patient is then asked to swallow at various levels in order to measure the strength and peristaltic movements of the muscles.
The whole procedure takes around 10 -15 minutes.
What is Anorectal manometry?
It is a procedure, used to measure the pressures in the anal sphincter muscles, which are responsible for initiating the sensation of bowel emptying in the rectum along with reflexes, needed to maintain normal bowel movements and mainly indicated in patients who present with constipation and fecal incontinence.
In some people who have weaknesses of the anal sphincter muscles, the normal mechanisms of tightening and relaxation on bowel movements will not take place properly, leading to a bowel incontinence which may result in leaking of bowel in an untimely manner and constipation respectively.
Procedure:
A complete history will be taken from the patient followed by a thorough explanation about the procedure and there after an informed written consent will be obtained.
Then, the patient will be asked to lie on the bed to his left side and a tiny flexible tube (like a thermometer) with a balloon attached to its end (catheter) will be inserted into the rectum.
Meanwhile, the machine which is connected to this catheter will measure the pressure in anal sphincter muscles. Also, by inflating the small balloon attached to the catheter, normal anal reflex pathways can be assessed in patients indicated.
More importantly, after the procedure, patient will be released and he will be able to engage in his normal lifestyle without any obstacle.
